Business_Finance_14061WAtoday.com.au reports:  As the realization dawns on just how close QF32 came to disaster, attention has quickly turned to Rolls-Royce, the world’s second-largest jet engine maker, and whether vital warnings signs had been missed.  What experts believe can be learnt from QF32 is the need for manufacturers, airlines and regulators to be willing to change their practices in step with the development of new aircraft.

”One of the things in aviation safety is ‘hindsight bias’. It is just so easy after an accident to say they should have known,” says Thomas Anthony, the director of aviation safety at the University of Southern California.  Airworthiness directives are issued regularly – in October the Europeans released 39 – and those relating to the Trent 900 did not make reference to the possibility of an uncontained engine failure.
